Request procedures for disclosure, notification of purpose of use, correction, suspension of use, suspension of provision to third parties, etc.
I. When requesting disclosure or notification of purpose of use
Please read the <Notes> below, attach the attached documents and a postage stamp equivalent to the fee to the designated request form, and mail it to the following office.
Please note that requests for disclosure or notification of purpose of use cannot be handled at any office other than the following office.

To confirm the authority of representation, please attach the documents a) and b) below, and to confirm the identity of the proxy, please attach one of the documents c) to f) below.- a"Power of attorney (please stamp with a registered seal)"

- 4Fees required for requests
Please affix a stamp equivalent to the fee to the request form. - 5How we respond to requests
Disclosure will be made by one of the following methods specified by the customer (including the proxy. The same applies below). However, if disclosure by the method specified by the customer would require a large amount of expenses or would be difficult to disclose by other methods, we will notify the customer and disclose by method a. Furthermore, if the customer does not specify a method of disclosure and has no objections, we will disclose by method b.In addition, for notification of the purpose of use, we will send a response document from our company to the customer's address (address stated in the attached document) by registered mail with delivery confirmation.- aBy issuing a written document

When we decide not to disclose all or part of the personal data or third-party provision records, when we decide not to notify the purpose of use, or when the relevant information does not exist, we will notify the customer without delay. - 6Cases in which disclosure or notification of purpose of use is not possible
Please note that we may not be able to respond to requests for disclosure or notification of purpose of use in the following cases.- aIf there are any deficiencies in the details or attached documents
- bIf the power of attorney cannot be confirmed (if the request is made by an agent)
- cIf required by law
- dIf the fee set forth in 4. above is not paid
- 7If the requested personal information is not held by our company
We will reply that we do not hold the requested personal information.
II. If you request correction, suspension of use, etc.
In accordance with the law, we will respond to the following window after confirming that the request is made by the customer (including an agent).

If you request correction, suspension of use, suspension of provision to a third party, etc., the following items from the <Notes> for requests for disclosure and notification of the purpose of use mentioned above will also apply mutatis mutandis.
